  2. It would be so helpful to have an icon on the appointment on the calendar page that shows if a client has NOT filled out all of their forms - rather than having to click into the client + their forms tab to see if they've filled them out every time.

  3. The current workflow for Medical Director chart review and sign-off in Boulevard is difficult to navigate and can be time-consuming. At the moment, there is no clear way to:
        •    Easily see which client charts still need to be completed by staff
        •    Identify which charts are pending Medical Director review and signature
        •    Track which charts have already been signed off

    Additionally, there is no automatic notification sent to the Medical Director when charts are ready for review and signature, which can lead to delays and confusion.

  6. This feature allows patients to automatically receive personalized post-treatment instructions immediately after they complete their checkout. Instead of relying on verbal reminders or paper handouts (which can be misplaced or forgotten), the system sends clear, standardized, and customizable aftercare instructions directly to the patient’s email or via text.   7. It would be helpful to send a form to a client at anytime without having a booking. When someone books for someone else, it takes time to cancel the booking, rebook under the other parties' name just to send the form.

  8. If the client's appointment is already confirmed, the Forms Reminder email should look different than the confirmation email. Including the subject line, it all reads like another confirmation email so clients will ignore it because they think "I've already confirmed," often times they don't think to scroll down and see that forms are missing. At the very least, the subject line should indicate the difference.

  11. It should be possible to make edits to charts after being completed, and the edits should be auditable. There should be a larger character limit as well than notes currently allow.

  15. We’ve found that when clients are asked to complete multiple forms, they often only finish one. This isn’t intentional—it’s usually the result of form fatigue and an overly complicated process.

    A simpler solution would make it easy to combine multiple forms into one, add an existing form to another, or copy and move sections between forms. This would reduce repetition, make the experience easier for clients, and save time for staff.

    Streamlining forms in this way would lead to higher completion rates, fewer follow-ups, and a smoother, more user-friendly experience overall.

  16. The current forms software is just not as inclusive as all of the others out there. Not being able to send a form separate from an appointment is very inconvenient. I have membership agreements and package agreements that aren't tied to specific appointments, but need to be filled out and kept in the clients profile. what is the point in having online forms if i have to keep separate forms for these things. it makes me want to cancel my forms subscription and use a seperate application. I should also be able to send a form to a designated ipad…

  17. Create a way for a provider to sign a form and then send to patient to "sign off" on like the Good Faith exam. Also enable review of forms signed on patient portal.

  19. Add camera grids and face alignment templates (like RXPhoto) so photos can be lined up the same way every time. This would allow consistent before/after pictures with preset angles (front, profile, 45°) and make comparisons more accurate.
