Ability to view completed forms as a client
Ability for clients to view their completed form submissions

I think in addition to this if they go in to their client portal now they see the forms have been completed but you can not click on them or have a button on the side that would open up a copy of their signed form

We need a way for clients to see the forms that they have signed such as pre/post information for repeat upcoming appointments

Allow patients to access signed forms instead of them not being accessible at all. This allows them to refer to post care instructions and informed consents at anytime. Having to save as PDF and then e-mail should not even be a thing. They should be able to access POST CARE instructions AFTER their appointment. This should be visible within their portal as well. Not just that they have been completed.

An option to send a follow up form/guidelines would solve this and be super helpful!

Our spa, like many others, sends out forms ahead of time so clients know what to expect. these include pre and post treatment forms. clients are reporting that once the form is signed, they cannot review them, even in boulevard client portal. this has become a waste of paper for our clinic, because we are constantly sending clients home with paper copies.

This would help them when having to redo annually

We need a portal to send discharge instructions through. We do IV's, modalities, and laser hair removal. Our patient's need a place to access these documents.

Agreed! We have a lot of pre and post instructions that we give our patients and they need to be able to go back through and open them.

Commenting to boost and say how CRITICAL this is for clients to be able to open and review the forms. They are consenting to procedures that they often want to be able to re-read about. We also use this for pre and post instructions, it is critical that clients are able to open and ready thru post-instructions.
As mentioned by someone else, any form filled out by a client should automatically be emailed to the client.

It would be nice for patients to be able to access their forms (pre & post treatment instructions) right in their client portal.

Commenting for more traction on this idea. Yes, as many have said, we send post-care instructions and the patients are not able to access them after their appointment is complete.

We need a way for clients to either view all signed forms in their portal to reference back on such as PRE AND POST CARE, or we need BLVD to email a copy of all signed forms automatically to patients after completion.

We have a post care form that I would like to add, but when the client "submits" the form, it would not be able to be reopened in the client portal. If they could still click into the form and be able to view without editing, this would be ideal. Thanks!

Just commenting again to discuss how critical this is.

These forms consist of pre- and post-treatment instructions! How can they follow their instructions when they are not available for them to see once they’ve completed the form?
If we are advertising BLVD as a medical platform in any way, there are so many improvements that need to happen to be able to call yourselves that. -

Can this please be reviewed for consideration? Printing out all this paperwork for clients is not what I expected to have to do when going with online charting and forms. Thanks!

Allow clients to recall forms after they have filled them out. This allows them to view pre and post care instructions and other information they have agreed to.

Our patients are requesting the ability to send and receive documents/attachments to our office securely through the patient portal.
It would also be helpful if patients could view a read-only version of documents they have previously signed (ie: post-treatment reminders).

The business wants/ need the clients to be able to see and access completed forms from within the client portal. As of right now, forms can only be submitted.
