Upload photos on intake forms
Per Glowbar: "We are looking to see if the intake form has a way to upload a photo so that clients can take pictures of their skin at its current state prior to their virtual consult. This would be extremely helpful to our appointments and allows for the best service to our clients."
Hey folks! Thank you for your feedback! Great news - we launched this feature last week to all forms customers, and we've already seen several folks adopt it with style! Now, anyone completing a form can upload an image, or if they're on a mobile device, they can capture the image directly in Boulevard. Check out our changelog announcement to learn more, or head to the support article to get started on setup!

I agree as well this would be very beneficial for clients to take photos of there previous permanent makeup or any concerns with skin, asymmetry and other issues relative to their booking. We are currently using an external form for this and would love it integrated into the boulevard system

I agree with others who've expressed an interest in having the option to upload an image in forms. In order to streamline our new client intake process, ideally clients should be able to upload a photo of their current hair so that we can effectively assess potential services prior to an appointment booking.

Would be very helpful to be able to attach an external document or photo into a patients chart for a specific appointment instead of in the overview section.

Image uploads are integral to our New Guest Intake process.
With Jotform we would have our clients upload photos of their goal and their hair currently, which helps the stylist prepare for the appointment.
We switched to Boulevard forms from Jotform, but this is a fatal flaw which might require us to switch back.

Partner would like to add photos to intake forms which would allow clients to select from various services.
