Adding addendums to completed charts
It should be possible to make edits to charts after being completed, and the edits should be auditable. There should be a larger character limit as well than notes currently allow.

We need to submit forms to insurance companies sometimes, so it is imperative that we are able to cleanly print the ENTIRE notes - including any addendums made - without overlap or cutoff

Allow staff to send forms to patients independent of an appointment.
I currently operate with the idea that I am able to send forms to patients independent of appointments such as is the case with month to month follow up for telehealth weight management. I want to be able to send a patient a follow up form through their portal so that they can complete for review and is automatically tied to their chart. I was able to do this with my previous EHR which is a game changer for our practice. We do not have access to forms in any other capacity and so I am currently utilizing google forms…

Improve Signature and Date Entry on Forms
We've noticed that the current signature and date entry fields on patient forms are not very user friendly, especially for our older clientele. Many patients struggle because:
- the buttons and options are small and hard to see/read
- there are multiple steps/buttons, which makes the process confusing especially for the signature part.it would be helpful if the signature/date section could be simplified and made more accessible - for example, large buttons, clearer instructions, and a more streamlined entry process. this would improve patient experience (many of them get frustrated and upset) and reduce time spent assisting clients with.…

boulevard needs to make a way for medspas to enter diagnosis codes and cpt codes. currently we dont even have a way to make that information show up in notes on a patient receipt.
Patients that wish to send to an insurance company on their own have to have this information.You also DEFINITELY need a way to make a non global area on a patient receipt to add notes for the patient to see

forms automatically send out forms that have been updated
It would be nice that if we update a form it automatically will trigger clients to fill out the new form at their next appointment. Right now the way the system is we have to either manually add for each appointment or have the form filled out every time for each client. Also if you make the form expire it should trigger to have clients fill out an updated form.

Pull information from profile for intake form
When we book a new client and enter all their contact information, the intake form makes the client fill out all of their contact information and address again making for a clunky and tedious experience for them.

Put not started charts under the "forms and charts" tab so that we can access it easily instead of having to click into the "view appt"
This prevents the extra steps to get into the chart when we are looking at the gallery and then having to chart from that page.

Forms
It would be very helpful to have a visible icon or indicator on each appointment to show whether a client has completed their required forms.
Currently, we have to click into each client’s profile to check if their forms are filled out. Having a quick visual cue (for example, a red alert icon if forms are incomplete and a green check if they are complete) would save time and help ensure all paperwork is in order before the appointment.

Please create a role that allows the editing and creating of forms and charts without giving complete admin access to all other reports and sensitive business operations.

chart sign off
It would be super helpful to have a way to be notified that a chart is not signed-off/finished. Especially, as a medical director, having to page through every chart individually to see if the nurse has the chart completed takes so much time!

Internal Only Form or Expiration on Charts
It would be extremely helpful to have internal-only forms — ones that are not visible to clients — so we can set up documents like medical authorizations with a built-in annual expiration date. Alternatively, if it's easier, enhancing the current chart functionality to include an optional expiration setting would allow us to manage these internal forms more effectively without exposing them to clients.

Upload photos to treatment plan from Gallery
When uploading a photo to mark-up in a treatment plan, there should be the ability to upload photos from the patient's Gallery, not just take a new photo/upload from camera roll

create a library
Create a library of forms/charts/phrases that people have created. Give them the option to add the form/chart to library to share with others. Create a button for others to share to the library so everyone doesn't have to build from scratch.

Make the placeholder text editable
as a medical office we use the same wording for alot of our charting but need to edit patient specific portions of the text. we use the placeholder when making our chart notes but would like it to be editable once were in the actual note using it. as it is set up right now it is an all or nothing text.
i know we could use the "commonly used" phrases you just started but there is a max of 1000 characters which isnt long enough for us.
overall it would be nice to have the placeholder be editable.

Intake form: even if patient signs off on paperwork, make the drop downs accessible for providers
Our clients often times do not complete their intake forms appropriately, so when i go over them with them the only way i can edit is by adding a note. Often times these notes are drop downs, which once "completed" by clients, the drop downs are not able to be accessed. Make it so that the not stays in its original form until the provider signs off on it.
