Memberships - Agreement Forms
Would like to be able to attach forms to a membership or series (product instead of service). The workaround for this now is not ideal at all.

Im desperate to see this one happen. Trying to find a work around for this has been a big headache. Hopefully going to be able to figure something out with docusign and zapier.

Is there any update on a status for this? It appears to be over 2 years old. This absolutely necessary in our medical spa space. We’re moving to 3 new tiers of memberships and the membership “product” functionality to do some is becoming extremely prohibitive to our practice. I am now exploring 3rd party software to manage memberships with all of these limitations within blvds platform. For the extremely high and increasing monthly cost of Blvd (which we love for almost everything else) there needs to be a major overhaul of memberships. This is a massive part of our revenue in the Med Spa space and its only growing at a rapid pace in the industry.

Yes, the client needs to be able to read the agreement when purchasing the membership -- especially online. The workaround of having to attach an intake form with the membership terms to their appointment doesn't work for online sales of memberships.

Could BLVD add the ability for clients to sign an acknowledgement/agreement regarding our membership policies during the checkout process when signing up for a new membership? We could keep a copy of the acknowledgement for or records and a copy of the agreement could be sent to the client's email.

Have the ability to manually send a form (not attached to a specific appt). For example, if a client decides to join a membership, we can manually send them the form.

This is CRUCIAL for online membership sales. The fact that we can not do it, puts us at high risk if we choose to still sell memberships online and the fact is that everyone in my area does sell memberships online so not being able to also creates a large impact on my business. Software should protect us - not leave gaping holes where we can be liable for huge refunds or worse yet litigation.

Would LOVE to have a way we could create a contract/agreement in forms and have the option of it popping up for the client to sign and save to their profile at the point of sale. This would consolidate many work arounds and make the process much more streamlined.

Some of these product sales are large ticket items that probably should have a form to show the clients the terms of purchase and then we have a record of such agreement so the business and boulevard are both protected in these big sales.

Currently, clients can only fill out forms if they are attached to an appointment. This causes an issue with membership agreement forms. Members are not able to fill out an agreement after their appointment when they choose to sign up for a plan. Please separate the forms from appointments and instead attach them directly to client profiles with the ability to be sent or filled out by the client at anytime

Can we update the FORMS capabilities to also include forms triggered by a membership (product) purchases. For example, when someone purchases a subscription/membership, they are promted to sign a form with the terms and conditions of the membership. Thanks!
