Forms not in appointment details page online
For Forms, if the same-day reminder and Duo app are the only options turned on, the form should not be available to sign in the Appointment Details page online,

When the "Reminder Text", Same-day Reminder, or at sign in with DUO setting is chosen for a form it should stop a forms from sending with the confirmation email
