Similar to how a staff's name appears on a purchase receipt, we want to see which staff member cancelled a client's membership

It's helpful to see who initiated a cancellation (staff or client) in the Membership SCHEDULED Cancellation report, but once the cancellation date has passed, visibility in reporting is lost altogether and can only be viewed in individual profiles. Adding the "Initiated By" column in the Membership Cancellation report helps keep that information available even after the cancellation date has passed.
