Form Update Once Per Year
After the intake form expires once per year certain information should auto populate so that a patient does not have to refill out the entire form just check off if anything has changed.

Boulevard should have a form that allows the patient to check a box every year that nothing on the intake form has changed. This form should have the ability to be launched one year from the date of the renewal of the intake form and every year after that.
