We need automated yearly health forms for facial clients. Right now, the option is every facial which is overkill or one time only.

A client health history or consent form having the ability to set an expiration date from 1st signature. We have to have an updated health history from clients every year not every appointment and we would like it to auto assign instead of having to manually doing it.

It would be great to be able to set an expiration date on Forms. For example, we complete a new GFE/Medical Eval every 12 months. We are currently tracking via making a note in the booking alerts, but it would be MUCH easier if we could set an expiration so that it automatically populates again when they're due.

Currently there is only a way to send forms for clients to fill out at their first appointment or every appointment. Intake or Medical History forms need to be filled out annually, so inputting an option for clients to be sent "Annually" or a year after they first completed.

Some of our forms need to be signed once a year but there is not currently a way to do this automatically. It would be helpful if there boulevard would know that a year has passed and automatically add this to a client's forms

Include a setting on Client Forms that tracks when the client hasn't completed a form in over a year an automatically send it to fill out. Med Spa businesses require newly updated forms every year.

It would be great to have patients fill out forms every X amount of time (yearly would be ideal). This way we don't have to duplicate new forms at the beginning of each year and require everyone to fill out even if they just filled it out in December (a month prior).

Medical establishments require their patients to fill their intake forms out yearly, not just one time forever. We need the option for forms to expire after one year since they were filled out so that our clients are automatically required to fill them out again after one year.

Since we offer massage therapy we are required to have clients update intake forms every year. It would be nice to add an option on the form for it to "expire" and prompt them to update via email or for their next reservation.
