    As a medical practice, I believe this needs to be viewed as more than a convenience or workflow issue. It is a medical-legal documentation issue.

    Patient name, date of birth, and the date of service are fundamental identifiers in a medical record and should automatically populate on clinical forms and consents that are associated with a specific patient and visit. Requiring patients or staff to manually re-enter this information creates unnecessary opportunities for omission, transcription errors, or a form being associated with the wrong date of service.

    This becomes even more concerning when a consent is later printed or exported for the medical record. The printed document should clearly identify who signed it and when it was signed, without relying on information that is visible only elsewhere in the electronic chart.

    Boulevard already has the patient profile and appointment information available in the system. For practices using Boulevard as part of the medical record, these identifiers should be carried automatically onto the actual form and preserved on the printed/exported version.

    At a minimum, Boulevard should support connected fields for patient first and last name, DOB, date of service, and ideally practice/location, and ensure that the signature date/time remains visible on the final printed consent.

    This should be considered a high-priority medical documentation and risk-management issue, not simply a feature enhancement.

    We're in conversations with Shawn about his issue here. Kudos to him for catching what might be a bug that we plan to fix. We've also been able to adjust Precision Scheduling's settings to better suit their preferences. Please reach out to support if you'd like to adjust how Precision Scheduling works for your business. Between the bug fix and settings adjustment, we're confident we'll end up somewhere that works well for them!
