Connected fields in forms
As a medical practice a patient's DOB is required. Please do the following;
1) Require Birthdate when patients book online for the first time thereby automatically pulling it into their Client Profile,
2) Change the Birthdate Forms field so it does not automatically default to "Today's Date" on the mini calendar selector (many patients are clicking on Today's Date accidentally),
3) Add a field that automatically calculates the Age based on Birthdate

With medical practices forms are considered part of the patient chart. Medical practices are required by law to have the patients full name, DOB, and Date on every single form/chart note in a clients record. Due to this, all of our forms for pre/post treatment have the required data elements to make our chart comply with legal requirements. UNFORTUNATELY - this makes our patients very frustrated because they are constantly being asked to put in their name, DOB, and date, over and over and over again.
It would be great if FORMS would auto populate with the linked fields information. The patient could still change it, but it would be fewer fields to populate.

Having an EMR that doesn't require a birthdate on their profile is not only inconvenient, it's dangerous. We have two clients with the exact same name and the only way we can identify them now is through their phone number. If you are going to try to be a legitimate EMR, requiring certain identifying data is a must. This is something that should be fixed yesterday. Using a form to gather and store the DOB is not a valid workaround as it requires too many clicks to see that vital piece of information.

OMG this is so freaking annoying!! I made all these forms with those fields with the "link" symbol and thought it would auto-populate from the chart into their chart and vise versa (or if they did it in one form it would cary over) but it does NOT. SO I removed all those fields from the chart and only have it on one questionnaire now because its ridiculous for the client to have to enter it in 5 times if they have multiple forms.

BioPeak would like the ability to have a connected field for "Current Medical Problems" and "Family History" so that they can auto-populate in a chart

We REALLY need patients to be required to enter their date of birth when booking ANY appointment - even if it is not their first. This would help minimize confusion when a parent is booking an appointment for a child, but using the parent's email address. We have had many parents book themselves instead of their kid because their email is already associated with their account, and the system cannot differentiate which patient it should book. USE DOB!!

Allow email to be a text input "connected field" in forms. So that guests can update their email and those guests who we do not have an email address for can enter it directly online PLEASE PLEASE. We stopped collecting emails on our last program for the last 4 years as we mostly had communicated through texting. We now have so many guests that we have imported to Blvd that have an out of date emai or don't exist at all. Taking email addresses over the phone and or on checkout is cumbersome and awkward and often they are input incorrectly. (and no we do not use the Duo Ipad on checkout) Doesn't fit for our demographic

The intake forms do not properly save clients date of birth and I get multiple complaints daily on how difficult it is the input your bday info & don't see any way to fix this on my end.
there needs to be an option to type in the date
