Booking questions for booking widget
Prior to booking, widget should prompt client to answer the following questions:
- Is this booking for you?
- If yes > normal booking flow
- If no > is this booking for a minor?

require certain demographics when booking patients, both online booking and front desk booking i.e. address, bday, email, phone number, etc

Also to be able to asking booking question one they select a service. Such as contraindications, what type of skin do you have, etc. This would be very helpful for the med spa industry.
