Medical establishments require their patients to fill their intake forms out yearly, not just one time forever. We need the option for forms to expire after one year since they were filled out so that our clients are automatically required to fill them out again after one year.

Setting that allows for forms to be sent out once annually for clients to fill out.

Since we offer massage therapy we are required to have clients update intake forms every year. It would be nice to add an option on the form for it to "expire" and prompt them to update via email or for their next reservation.

Archiving a form should activate the requirement for a new form. We require massage forms to be completed within one year, when someone completes the form online we are never able to have them resubmit that form online. Archiving the form should trigger the system to require a new form. This would allow us to have them complete the form online for their next appointment rather than having to come in and complete it in paper form.
