add "reason for change/cancellation" when clients self-cancel or self-reschedule
currently we can only note this if the provider cancels for the client, but we want to know why clients change their appts when they change it on their own

I would love for when clients cancel on their own the system to prompt them to a drop down of reasons for canceling such as "sick" "no longer happy" "rescheduling" etc.
